Legendary Basketball Academy — Waiver and Release of Liability
I, the undersigned parent or legal guardian of the above-named minor ("Participant"), hereby acknowledge and agree to the following in consideration of the Participant being permitted to participate in the Legendary Basketball Academy Summer Basketball Camp ("the Camp"):
1. Assumption of Risk. I understand that participation in basketball training, drills, scrimmages, and related physical activities involves inherent risks of physical injury, including but not limited to sprains, fractures, concussions, heat-related illness, and other injuries. I voluntarily assume all such risks on behalf of the Participant.
2. Waiver and Release. I, on behalf of myself, the Participant, and our respective heirs, assigns, and representatives, hereby release, waive, and discharge Legendary Basketball Academy, its coaches (Lee Sanders, Kristopher Sanders, and Austin Sanders), staff, volunteers, and affiliates from any and all liability, claims, demands, or causes of action arising out of or related to any injury, illness, or damage sustained by the Participant during or as a result of participation in the Camp, except in cases of gross negligence or willful misconduct.
3. Medical Authorization. In the event of an emergency, I authorize the Camp staff to obtain emergency medical treatment for the Participant. I understand that I am responsible for all costs of such medical treatment. I have provided accurate information above.
4. Photo/Video Release. I grant Legendary Basketball Academy permission to photograph or record the Participant during camp activities and to use such images for promotional purposes, social media, and the academy website.
5. Camp Rules & Code of Conduct. I understand that the Participant is expected to follow all camp rules, show respect to coaches, staff, and fellow campers, and demonstrate good sportsmanship at all times. I understand that Legendary Basketball Academy reserves the right to dismiss any participant for behavioral issues, without refund.
6. Personal Belongings. I understand that Legendary Basketball Academy is not responsible for lost, stolen, or damaged personal belongings.
7. Lunch & Nutrition. I understand I should pack a small lunch for my camper. Lunch may be provided for an additional fee.
8. Camp Swag. I understand that camp backpacks, camp shirts, and a certificate of completion will be provided at the end of camp.
This waiver shall be governed by the laws of the State of Georgia. I have read this waiver in its entirety and understand that by signing below, I am giving up substantial legal rights.
